If you have no medical insurance and are not eligible for Medicaid:
We offer affordable health care through the use of a Discount Fee Program to all patients at or below 200% of the poverty level. All patients are eligible to apply for the Discount Fee Program.
If you want to apply for our Discount Fee Program, you will meet with a Patient Services Representative who will review your application and all paperwork. At this time, the Representative can answer any questions you may have regarding the Discount Fee Program and how it works.
If you are applying for the Discount Fee Program, please complete the Discount Fee Program Application Form (see link to the form, at right). You will be required to bring to the Clinic proof of identification and proof of ALL income that is received in your household.
If your income indicates that your household is at or below 138% of the poverty level, you may be required to apply for Medicaid and submit a Medicaid Denial Letter to us prior to using our Discount Fee Program.
